Behavior Targeted:
The behaviors targeted for intervention include tantrums, aggression, and non-compliance, as outlined in the individual treatment plan.
Goals & Expectations:
The goal is to reduce the frequency of disruptive behaviors by 50% over three months, utilizing ABA techniques as specified. All parties agree to cooperate and communicate openly.
Responsibilities of Behavior Analyst:
The behavior analyst will implement evidence-based interventions, monitor progress, and communicate regularly with the client and family regarding updates and modifications.
Responsibilities of Client/Parent:
The client/parent agrees to follow the recommended intervention plan, attend scheduled sessions, and provide honest feedback on progress and challenges.
Confidentiality & Legal:
All information shared will be kept confidential, and this agreement will be governed by applicable laws regarding healthcare and data privacy.
Additional Provisions:
