Sample Co-Parenting Contract

Parties Involved:

Parent 1: Jane Doe

Date of Birth: January 15, 2018

Custody & Visitation:

Parents agree to shared legal custody and joint physical custody, with visitation schedules as detailed herein or as amended by mutual agreement.

Parental Responsibilities:

Both parents commit to co-parenting in the best interests of the child, including participation in decisions related to education, health, and welfare.

Child Support Payments:

Parent 1 shall provide financial support in the amount of $500 monthly, to be paid on the 1st of each month, for the child’s expenses.

Dispute Resolution:

Any disagreements arising from this agreement shall be addressed initially through mediation, with legal proceedings as a last resort.

Amendments:

This agreement may be modified only in writing signed by both parents.
